Father's Day Questionnaire

With father's day being only 4 weeks away, I thought I would share with you what my students and I prepare each year for our Dads. This is great to do with even very young children!

Make a list of the following:

My dad's favorite animal is:
My dad's favorite thing to do is:
My dad's favorite sport is:
My dad's favorite song is:
My dad's favorite movie is:
I know my Dad loves me because:

Have your child answer these questions and then fill in your child's answer on the sheet. You may add more questions of your own to this list. Your child may add a painting or hand print below the questions.

This is so fun, because the answers are hilarious!
